Eden Health - - Responsibilities: Provide in-home physical therapy services under the physician's plan of care for patients on caseload; Review caseload with supervising Physical Therapist on a regular basis; Complete accurate and thorough visit notes in the Electronic Health Record for every patient; Educate patients, families/caregivers, and staff on physical therapy modalities; Identify and mitigate environmental/safety/mobility risks in patients' homes
