Mountain View Hospital is looking for a Social Worker Discharge Planner to join our team!
Job Summary:
A hospital social worker/discharge planner supports patients and families by coordinating safe discharge plans, connecting them with community resources, and addressing social, emotional, and practical needs during hospitalization.
Key Responsibilities:
Patient assessment — Evaluate patients' social, emotional, financial, and support needs to identify barriers to discharge.
Discharge planning — Develop individualized discharge plans, arrange follow-up care, and coordinate services such as home health, rehabilitation, or long-term care.
Resource coordination — Connect patients and families with community agencies, financial assistance, transportation, and support programs.
Interdisciplinary collaboration — Work closely with nurses, physicians, case managers, and therapists to ensure smooth transitions of care.
Patient advocacy — Ensure patient rights are upheld and help families navigate complex healthcare systems.
Documentation — Maintain accurate, timely records of assessments, plans, and interventions.
